In order to see a provider, all clients must first be registered.

Note: (Sept. 1, 2022) At this time, we are only conducting new registrations for Native American clients.

Call 707-485-5115 to schedule a time to complete the registration packet and apply for coverage. They will help determine if clients who are American Indian or Alaska Native qualify for Purchased & Referred Care (PRC). These meetings are at PRC's registration office in the main lobby of Medical Building.

Client Registration
All new clients and clients who have not used CTHP services in the last three years must register before accessing services. Pick up a hard copy of the registration packet on campus, print a copy from the website, or request that a packet be mailed to them.

To return a completed packet:
1) Bring the packet to the Medical Building Lobby
2) Fax the packet to 707-467-5626, or
3) Mail it to: CTHP

Insurance Coverage
Clients who are uninsured please contact PRC at 707-467-5608 for assistance. Registered clients who have PRC questions should visit their main office which is located in the lobby in Medical Building. Find out more on About Purchased & Referred Care (PRC).
